Victim Identified in Fatal Crash on Interstate 81 in Schuylkill County



A three-vehicle crash on Interstate 81 South claimed the life of a Shenandoah man and injured two others Friday evening, according to a Pennsylvania State Police at Frackville.

The crash occurred around 5:49 PM on November 14, 2025, near mile marker 135.5 on I-81 South. Troopers say the incident involved three vehicles traveling southbound: a 2024 Honda Civic, a 2024 International tractor-trailer, and a 2016 Audi Q5.

According to investigators, the chain-reaction crash began when a Honda Civic driven by Rafael Ceballos, of Hazleton, re-entered the right travel lane after passing another, uninvolved vehicle that was stopped on the west shoulder. As Ceballos merged back into the lane, the front right side of his vehicle struck the rear left side of an International tractor-trailer driven by Charvel Prophete, of Columbus, Ohio.

After the initial impact, the Honda then struck a 2016 Audi driven by Danyel Miller, of Shenandoah, which had been traveling in the left southbound lane. Troopers say the Honda spun multiple times before coming to rest in the right lane of I-81 South, facing north.

The Audi left the roadway, traveled down the embankment in the grass median, and rolled several times before coming to rest in the left lane of I-81 North, facing east.

The tractor-trailer was able to come to a controlled stop on the west shoulder of I-81 South.

Miller, the driver of the Audi, suffered fatal injuries and was pronounced deceased at the scene by the Schuylkill County Coroner’s Office.

Ceballos and Prophete both suffered suspected minor injuries.

Ceballos was transported by McAdoo Fire Co. Ambulance Association to Lehigh Valley Hospital–Hazleton.

Prophete was transported by Shenandoah Community Ambulance Association to Lehigh Valley Hospital–Schuylkill East.

All three vehicles sustained disabling damage, except the tractor-trailer, which received minor rear-side damage.

The Pennsylvania State Police–Frackville barracks is continuing to investigate.

All three vehicles were towed from the scene by Mark Hammer’s Towing.
